1. ভর্তির আবেদনের জন্য শিক্ষার্থীর উচ্চমাধ্যমিক এবং মাধ্যমিকের তথ্য,
2. বর্তমান ঠিকানা ও মোবাইল নম্বর, পিতা ও মাতার জাতীয় পরিচয়পত্র নম্বর (ঐচ্ছিক)।
3. শিক্ষার্থীকে ৮ টি বিভাগীয় শহরের যেকোন ১ টি কে তার ভর্তি কেন্দ্র হিসেবে নির্বাচন করতে হবে।
4. স্ক্যান করা একটি ছবির (Format: jpg, Size: 30 – 200KB, Width: 360-540px, Height: 540-720px) প্রয়োজন পড়বে।
5. SMS করার জন্য শিক্ষার্থীর কাজে টেলিটক, রবি, এয়ারটেল অথবা বাংলালিংক অপারেটর এর একটি মোবাইল নম্বর থাকতে হবে।
6. ভর্তির আবেদন ফি অনলাইনে মোবাইল ব্যাংকিং , ইন্টারনেট ব্যাংকিং বা চারটি রাস্ট্রায়ত্ব ব্যাংকে (সোনালী, জনতা, অগ্রণী ও রূপালী) নির্ধারিত সময়সীমার মধ্যে জমা প্রদান করা যাবে।
